Testimonial for medical indication

A testimonial for medical indication is a written statement by a general practicioner, in which an impression based on medical data is given concerning the patient and his or her health with another purpose than treatment.

The Royal Dutch Medical Association (KNMG) recommends treating general practicioners not to give a testimonial for medical indication concerning one's own patients. However, a treating general practicioner may, with the patient's permission, provide factual medical information.
