It may occur that various kinds of authorities ask you for a testimonial for medical indication.
A testimonial for medical indication is a written statement by a physician, in which an on medical data based impression concerning the patient and their health is given with another purpose than treatment.
The physician federation KNMG (Royal Dutch Medical Association) recommends physicians not to give a testimonial for medical indication concerning one's own patients because they can not give an objective and independent opinion. However a treating physician can provide, with permission of the patient, factual medical information.
The KNMG has drafted a refusal statement which is considered to be leading by our GP practice.
You can download this refusal statement via the website of the KNMG so you can, if desired, submit it to a requesting authority.
